What ICBC pays, what a user fee is, and exactly where a Launch Rehab client does and does not pay something.

THE SHORT ANSWER
How much does ICBC physiotherapy cost in BC?
ICBC pays the clinic $97 per standard physiotherapy session for treatments between April 1, 2026 and March 31, 2027. Clinics may charge a user fee on top, which ICBC never reimburses and which the clinic must disclose in advance. At Launch Rehab, pre-approved physiotherapy, chiropractic, kinesiology and counselling sessions cost the client nothing, while registered massage therapy carries a surcharge of $7.45 to $7.65 per session.
Cost after a crash is where clinics tend to be vaguest, usually by saying ICBC covers it and leaving the detail for the front desk. The rules are clear enough to state plainly.
ICBC pays a set amount per session. Clinics are permitted to charge a user fee above that amount, and ICBC will not reimburse it. Whether you pay anything therefore depends on the clinic you choose, not on your claim.

WHAT ICBC PAYS
ICBC's session rates for 2026 to 2027
ICBC publishes a fixed amount it pays per treatment session, along with a minimum session length. These are the rates for treatments provided between April 1, 2026 and March 31, 2027.
Physiotherapy
$97 per session, 20-minute minimum of one-to-one time.
Chiropractic
$77 per session, 15-minute minimum.
Registered massage therapy
$107 per session, 45-minute minimum.
Kinesiology
$96 per session, 45-minute minimum.
Acupuncture
$115 per session, 20-minute minimum.
Counselling
$160 per session, 50-minute minimum.
Psychology
$246 per session, 50-minute minimum.

USER FEES
What a user fee is, and why clinics charge them
A user fee is an amount a clinic charges you directly, above what ICBC pays. ICBC's own program guide defines it as fees charged to a customer that exceed the regulated rates for standard treatment sessions, and states plainly that they are not reimbursable.
The guide also requires the clinic to tell you in advance, and clinics must notify ICBC when they start or stop charging one. So a surprise fee at the end of a course of treatment is a failure of disclosure, not standard practice.
Clinics charge them when ICBC's rate sits below their standard price for that appointment length. That gap is real in some disciplines and absent in others, which is why a clinic might charge on massage and not on physiotherapy.
It is a fair and reasonable question to ask any clinic before booking: do you charge a user fee for ICBC sessions, and on which treatments? A clinic that will not answer directly is telling you something useful.
AT LAUNCH REHAB
Exactly what you pay here
Pre-approved physiotherapy, chiropractic, kinesiology and clinical counselling sessions cost you nothing. No user fee, no per-visit charge, nothing at the desk.
Registered massage therapy carries a surcharge of $7.45 to $7.65 per session. ICBC funds RMT at $107 for a 45-minute session, and our standard 45-minute massage rate is $120 including GST as of April 1, 2026. The surcharge covers that gap.
We state this here rather than at the front desk because a $7.50 surcharge disclosed upfront is a minor detail, while the same amount discovered on arrival feels like something was hidden.
ICBC is billed directly at all five studios: Lougheed, Coquitlam, Richmond, New Westminster and North Burnaby. You do not pay and claim back for pre-approved sessions once your claim number is on file.
AFTER 12 WEEKS
What happens to cost when the window closes
Once the Early Access Period ends, treatment is no longer automatically pre-approved. Further ICBC funding requires a request supported by clinical reasoning, and it is not guaranteed.
If continued funding is approved, the same rate structure applies. If it is not, there are other routes. Most extended health plans cover physiotherapy and massage, and Launch Rehab direct bills most major insurers including Pacific Blue Cross, Sun Life, Manulife, Canada Life, Green Shield, Desjardins and ClaimSecure.
MSP supplementary benefits are also worth checking. For people who qualify on income, MSP contributes toward a limited number of physiotherapy, massage and chiropractic visits per calendar year. At Launch Rehab, MSP-eligible physiotherapy rates are $147 for an initial assessment and $82 for a subsequent visit as of April 1, 2026.
Private rates, if neither applies, are $170 for a 60-minute initial physiotherapy assessment and $105 for a 30-minute subsequent visit as of April 1, 2026.
WORKED EXAMPLES
What a course of treatment actually costs
Three illustrative cases using the April 2026 to March 2027 ICBC rates and Launch Rehab's published prices, showing what appears on the ICBC invoice and what you pay at the desk.
A straightforward neck strain treated with weekly physiotherapy for eight weeks uses 8 of the 25 pre-approved sessions. ICBC pays the clinic $776. You pay nothing. Seventeen sessions go unused, which is the normal outcome rather than a wasted entitlement.
A more persistent case using 12 physiotherapy sessions and 6 massage sessions across the window bills ICBC $1,164 for the physiotherapy and $642 for the massage. You pay nothing for the physiotherapy and between $44.70 and $45.90 in total surcharges across the six massage appointments.
Someone with driving anxiety alongside neck pain using 10 physiotherapy and 8 counselling sessions bills ICBC $970 and $1,280. You pay nothing for either, since counselling carries no user fee here. The counselling allowance is separate, so using it did not consume a single physiotherapy session.

BEFORE YOU BOOK
Five questions worth asking any clinic
Billing practices vary more between clinics than most people realise, and almost all of the variation is legal. The differences show up in what you pay and in how much one-to-one time you get. These five questions surface it in about a minute.
Do you charge a user fee for ICBC sessions, and on which treatments?
The single most useful question. ICBC requires clinics to disclose user fees in advance, so asking is routine. A clinic that will not answer plainly has told you how the rest of the billing will go.
How long is the appointment, and how much of it is one-to-one?
ICBC sets a minimum of 20 minutes of one-to-one time for a funded physiotherapy session. Some clinics run longer. Time spent alone on a machine or a heat pack does not count toward that minimum.
Will I see the same practitioner each visit?
Continuity matters clinically, and it matters again at week 12, because an extension request depends on someone being able to describe your trajectory over time.
Do you bill ICBC directly?
Direct billing means you do not pay upfront and wait for reimbursement on pre-approved sessions. Most clinics do, but confirm rather than assume.
What is your cancellation policy for ICBC appointments?
ICBC permits clinics to charge no-show and late cancellation fees to customers, and does not reimburse them. Knowing the notice period in advance costs nothing.
WHAT THE FEE BUYS
Session length, and why two clinics differ at the same price
ICBC pays a fixed amount per session and also sets a minimum amount of one-to-one time that session must include: 20 minutes for physiotherapy, 15 for chiropractic, 45 for massage therapy and kinesiology, 50 for counselling and psychology, and 20 for acupuncture.
The important word is minimum. Two clinics can bill ICBC the identical $97 for a physiotherapy session while delivering quite different appointments, one running 20 minutes and another 30 or more. ICBC's guidance is also clear that the time counted is one-to-one time with the practitioner, and does not include periods when you are left unsupervised.
This matters practically. An appointment where 15 minutes is spent alone under a heat pack or on a machine is a shorter appointment than the schedule suggests, and after a crash the assessment and hands-on work are the parts that move recovery along.
It is a reasonable thing to ask before booking anywhere: how long is the appointment, and how much of that is one-to-one with the practitioner. The answer explains more about value than the headline price does, particularly when the headline price to you is zero either way.

WHAT PEOPLE GET WRONG
Three costly misunderstandings about ICBC billing
The first is treating free as a property of the claim rather than of the clinic. ICBC pays a fixed amount per session everywhere in BC. Whether you pay anything on top is entirely a clinic decision, so the same claim produces different out-of-pocket costs at different addresses.
The second is assuming extended health can top up an ICBC-funded session. It cannot. While ICBC is paying for a pre-approved treatment, that session is already covered and there is no remaining balance for an insurer to reimburse. Extended health becomes relevant when the pre-approved window closes, not alongside it.
The third is reading an out-of-date rate from a clinic website and planning around it. ICBC resets its entire fee schedule every April 1. A page quoting the previous year's physiotherapy rate is not wrong by a little, it is describing a schedule that no longer exists. That is why every figure on this page carries the period it applies to.
