OVERVIEW

What physiotherapy does for low back pain

Low back pain is one of the most common reasons people see a physiotherapist, and for most people it is not caused by serious damage. In many cases no single structure can be named, and the pain settles as the back is loaded and moved again in a graded way. Sciatica is different: it is leg pain, numbness or weakness that follows a nerve path down the buttock and leg, usually because a nerve in the low back is irritated. The two often overlap, and the first job is to tell them apart.

Physiotherapy is a regulated health profession. In British Columbia every physiotherapist is licensed by the College of Health and Care Professionals of BC (CHCPBC) and works to a defined scope of practice. A physiotherapist can assess your back, screen for the small number of problems that need a doctor, and build a plan to reduce pain and get you moving again. We do not diagnose disc damage on the page or promise a fixed timeline. Your therapist sets realistic expectations after the first assessment.

At Launch Rehab a session for back pain is an assessment, a hands-on treatment where it helps, and an exercise plan you can actually do between visits. We use manual therapy (joint mobilization and soft-tissue work), IMS where it is indicated, and graded active exercise, which is the part that changes how the back tolerates load over the following weeks. The same physiotherapist who assesses you is the one who treats you and progresses the plan.

Assessment

We take your history, screen for the red flags that need a doctor, test movement, strength and the nerves in the leg, and work out whether this is mostly back pain, mostly nerve pain, or both. You leave the first session with a working picture of what is driving it and a written plan.

Hands-on treatment where it helps

Manual therapy such as joint mobilization and soft-tissue work can settle a painful, guarded back and make movement easier. IMS (dry needling) is added when an IMS-certified physiotherapist judges it will help. Hands-on work supports the exercise plan, it does not replace it.

Active exercise and loading

Graded exercise is the part that changes how the back tolerates load over the following weeks. We start at a level your back accepts, then build strength and movement step by step. This is a short, specific program you can do between visits, reviewed and progressed each time.

WHY IT HAPPENS

What causes low back pain and sciatica

The low back is built to bend, lift and twist, and it is strong. Most back pain is not a sign that something is broken. It usually shows up when the back is loaded more than it is used to, or moved in a way it has lost tolerance for, often after a period of less activity. Pain can be real and sharp without there being damage, and understanding that changes how you respond to it.

Sciatica is a separate problem that can happen at the same time. It is nerve-related leg pain, which behaves differently from back pain and needs its own testing. The common drivers we see are:

  • Non-specific low back pain. The most common kind. No single structure can be named, and it usually settles as the back is loaded and moved again in a graded way.
  • A sensitive nerve (sciatica). A nerve in the low back gets irritated and refers pain, numbness or weakness down the leg. It follows a path rather than staying in the back.
  • Facet joints and muscles. The small joints and the muscles of the back can become painful and guarded, often after an awkward movement or a busy few days.
  • Load and deconditioning. A back that has done less for a while tolerates less. Pain often appears when activity jumps up faster than the back has been prepared for.
  • After a car crash. A collision can strain the low back and neck together, and ICBC covers the physiotherapy that follows, with no referral needed.

IN A FLARE

What to do when your back flares up

While you wait to be seen, a few simple things settle many sore backs. None of these are a cure, and none should sharply increase your pain. They lower the load so the back has a chance to calm down and you can keep moving.

  • Keep gently moving. Short, frequent walks and easy position changes are usually better than lying still for long stretches. Movement your back accepts is the goal, not pushing into sharp pain.
  • Change position often. Long sitting stiffens a sore back. Stand, walk a little, and shift how you sit through the day rather than holding one posture for hours.
  • Find easing positions. Many people find a bit of relief lying on their back with the knees bent and feet supported, or on their side with a pillow between the knees. Use what settles yours.
  • Warmth for muscle guarding. A warm pack on the low back for 10 to 15 minutes can ease muscle tightness. Use whatever helps you move more comfortably.
  • Ease back into activity. As the worst settles, build your activity back up in steps rather than jumping straight to what set it off. A flare is information your therapist uses to adjust load, not a setback.

This is general information, not a treatment plan or a diagnosis. Stop any movement that sharply increases your pain or sends symptoms further down the leg, and get urgent care for the red-flag symptoms listed above. The right exercises for you depend on what your assessment finds.

SCANS AND SURGERY

Do you need a scan or an operation?

Two questions come up in almost every back pain visit: do I need an X-ray or MRI, and will I end up needing surgery. For most people the answer to both is no, and knowing that early takes a lot of worry out of recovery.

  • Imaging is not routine. Most low back pain does not need a scan. Early imaging often shows normal age-related changes that are also seen in people with no pain, and it does not usually change the plan.
  • When a scan does help. Imaging is useful when the assessment points to a specific problem, such as significant nerve compression, or when red flags are present. Then it guides the next step, and your therapist will refer you on.
  • Surgery is the exception. Most back pain and most sciatica improve without surgery. It is considered in a minority of cases, usually significant nerve compression with weakness that is not improving, and that call belongs to a surgeon.
  • Physiotherapy is first-line. For most non-specific low back pain and most sciatica, active exercise and graded loading are a sensible first step. We tell you plainly if your case should be reviewed by a doctor instead.

COVERAGE IN BC

What back pain physiotherapy costs and what covers it

In British Columbia you do not need a referral to book physiotherapy. Current session prices are on our rates page. How the visit is paid for depends on how your back pain started and what coverage you have.

  • Extended health insurance. Most plans cover physiotherapy. We direct-bill most insurers up to your plan limit, and you pay any balance on the day.
  • ICBC (after a car crash). If your back pain follows a collision, it is treated within ICBC's pre-approved physiotherapy, with no referral needed. Bring your claim number and we bill ICBC directly.
  • MSP. MSP helps only with a limited number of physiotherapy visits for people on its supplementary benefits (lower-income households). It is not a general physiotherapy benefit.
  • WorkSafeBC. If your back injury is work-related and the claim is accepted, physiotherapy is covered at no direct cost to you.

A collision can strain the low back and neck together, so back pain, stiffness and sciatica-type leg pain often show up in the days after a crash. This is treated within ICBC's pre-approved physiotherapy, with no referral needed. Bring your claim number and we bill ICBC directly.

No. In British Columbia you can book a physiotherapist directly, without a referral. Most low back pain does not need a scan, and imaging early on often shows normal age-related changes that do not match the pain. Your therapist screens for the few situations that do need a doctor or imaging first, and will refer you on if that is the case. A small number of extended-health plans still ask for a physician's referral before they reimburse, so it is worth checking your own plan.
