CHRONIC PAIN GUIDE · APPROACH
Making sense of pain that has stayed.
An explanation of why pain can continue after tissue healing is finished, and what that means for the plan.

THE SHORT ANSWER
What is pain neuroscience education?
Pain neuroscience education, sometimes shortened to PNE, is a short, plain-language explanation of how chronic pain actually works. The focus is on how the nervous system can keep producing pain after tissue healing is finished, and why that does not mean the pain is imagined or that activity is dangerous. It is used as a first-line treatment component alongside graded activity in the 2021 NICE guideline for chronic primary pain, and in programmes like Pain BC's.
People with chronic pain are often told the scan is clean and the tissues have healed. For many, that answer feels like being told the pain is in their head. Pain neuroscience education is the response to that. It explains why pain can be real, physically felt, and no longer tracking tissue damage. The usual outcome is not that pain disappears in the room; it is that fear of movement drops enough for a graded activity plan to actually get used.

WHY PAIN PERSISTS
Pain is a signal, and signals can get louder
Pain is produced by the nervous system in response to a threat signal. In an acute injury, the signal usually tracks tissue damage closely. In chronic pain, after months of guarded movement, poor sleep, worry and reduced activity, the system often turns the signal up: smaller inputs produce bigger pain. This is a real, measurable change in the nervous system, not an imagined one, and it is why a clean scan does not mean the pain is not there.
WHAT THIS CHANGES
Understanding the signal changes what you do with it
People who understand that pain and tissue damage have decoupled are less afraid of movement. Fear of movement is one of the strongest predictors of chronic pain disability, and it is also one of the most changeable. Pain education is not a replacement for a graded activity plan. It is what makes the graded plan usable: without it, every twinge is read as damage and the plan gets abandoned the first week.
WHAT IT IS NOT
This is not 'the pain is in your head'
A common and understandable concern is that pain education is a polite way of saying the pain is not real. It is not. The nervous system changes that drive chronic pain are physical. They can be studied, measured and influenced by activity, sleep, education and sometimes medication. Pain education is about describing the physical reality more accurately so the plan matches it, not about asking you to think your way out of pain.

FAQ
Common questions.
Every physiotherapist on the team is trained to deliver pain education as part of a chronic pain visit. It is not a separate appointment or an upcharge; it is a short piece of a normal physiotherapy session, often five to fifteen minutes, repeated as needed over the first few visits. All five Launch Rehab studios (Lougheed, Coquitlam, Richmond, New Westminster and North Burnaby) deliver it under general physiotherapy.
