Do You Need a Referral for Physiotherapy in BC? ICBC, MSP, WorkSafeBC and Insurance
At Launch Rehab you can book physiotherapy without a doctor's referral. Here is what ICBC, WorkSafeBC, MSP, hospital programs and extended health plans each say, and the one case where a referral still matters.
BY THE LAUNCH REHAB TEAM
If you have a sore knee, a stiff neck or a recent crash behind you, you may be wondering whether you need to see your doctor before you can book a physiotherapist. The answer depends on who pays for the visit. This post goes through each payer in BC and tells you what its own pages say, so you know what to bring to your first appointment.
At Launch Rehab you book directly
You can book physiotherapy at any Launch Rehab studio without a doctor's referral. The same applies to registered massage therapy, chiropractic and kinesiology. You pick a studio and a time on our online booking page, and you do not need a note from anyone first. World Physiotherapy calls this direct access, which it defines as people being able to refer themselves to a physiotherapist without having to see anyone else first.
At Launch Rehab the booking needs no referral. Whether the party paying you back wants one is a separate question, and the next sections cover each payer.
ICBC: no referral for the first 12 weeks
If you were injured in a crash, ICBC's page on treatment in the first 12 weeks of recovery says you do not need a referral from a family physician or nurse practitioner for the pre-approved treatments, and you do not need approval from ICBC. You do need your Personal Health Number and your ICBC claim number, which is created when you report the claim. The page lists 25 approved treatments for physiotherapists and for chiropractors, and 12 each for registered massage therapists, kinesiologists, psychologists, counsellors and acupuncturists.
Bring the claim number to your first visit, because the clinic may need it to bill ICBC directly. Our post on the ICBC claim number and your first physiotherapy visit walks through what happens at the desk, and the ICBC physiotherapy hub covers the weeks after the first 12.
WorkSafeBC: your claim and your employer set the path
For an injury at work, treatment is tied to your WorkSafeBC claim. WorkSafeBC's Early Access to Physiotherapy Program page says that under the program the employer refers the worker for an initial assessment at a clinic that WorkSafeBC has pre-approved. The plain-English EAPP guide explains who qualifies, and the WorkSafeBC physiotherapy guide covers the first appointment.
MSP supplementary benefits: the page is silent on referrals
MSP supplementary benefits pay toward physiotherapy only for people who qualify. The BC government's supplementary benefits page describes a combined limit of 10 visits each calendar year across several therapies and a payment of $23 per visit. The page does not say whether a referral is required. Health Insurance BC answers that question, on (604) 683-7151 in the Lower Mainland. Our MSP physiotherapy coverage guide explains who qualifies.
Extended health plans may want a referral before they pay you back
Private insurance is the case that catches people. HealthLink BC lists "Do you need a referral from your doctor?" as a question to ask before you choose a physiotherapist and adds that some provinces and private insurance companies require one. At Launch Rehab, the clinic does not ask for a referral, and some extended health plans still require one before they reimburse a claim. Your plan booklet or your insurer's member line will say. Our post on extended health benefits for physio, RMT and chiropractic lists the questions to put to your insurer.
Hospital outpatient programs do ask for a referral
Public programs run through health authorities follow their own rules. HealthLink BC's listing for the physiotherapy program at Burnaby Hospital, run by Fraser Health, states "Physician or health care provider referral required. Services may vary per location." That listing covers a hospital program. If your surgeon offered you a place in a hospital program, ask that program how referrals work.
When to see your doctor before you book
Some symptoms need a doctor first and a physiotherapist later. Numbness or weakness that is getting worse, a fever with back pain, new trouble controlling your bladder or bowel, or chest pain with shortness of breath are same-day calls to your physician, or 911 if the symptoms are severe. A physiotherapist screens for these at every first visit, and will send you back to your doctor if the findings point there.

Frequently asked questions
Can I book a Launch Rehab physiotherapist without seeing my doctor first?
Yes. Launch Rehab does not require a referral for physiotherapy, and you can book directly online at any of the five studios. Bring your Personal Health Care Card number and a list of your current medications to the first appointment, plus a doctor's note only if you already have one. The physiotherapist screens your symptoms at that visit.
Do I need a referral for massage therapy, chiropractic or kinesiology at Launch Rehab?
No referral is required to book registered massage therapy, chiropractic or kinesiology at Launch Rehab. You can self-refer for each of them online. The same payer rules described above still apply: your extended health plan may ask for a referral before it pays you back, so check your plan booklet if you plan to claim.
Does ICBC require a referral for physiotherapy after a crash?
No, not for the pre-approved treatments in the first 12 weeks. ICBC's page says you do not need a referral from a family physician or nurse practitioner and do not need approval from ICBC. You do need your Personal Health Number and ICBC claim number. After the first 12 weeks, further treatment follows ICBC's extension process, which has its own steps.
Does MSP supplementary benefits need a referral for physiotherapy?
The BC government's supplementary benefits page does not state a referral rule. It does describe who qualifies, a combined limit of 10 visits per calendar year across several therapies, and a payment of $23 per visit. Health Insurance BC can tell you whether a referral applies to your situation, and its Lower Mainland number is (604) 683-7151.
How do I find out if my extended health plan wants a referral?
Read the physiotherapy section of your plan booklet, or call your insurer's member line and ask whether physiotherapy claims need a doctor's referral and whether there is a visit limit or a cap per visit. Launch Rehab says on its FAQ page that it can advise from what it sees from your insurer, so you can also call the studio before your first visit.
Do I need a referral for hospital outpatient physiotherapy?
Often yes. HealthLink BC's listing for the physiotherapy program at Burnaby Hospital says a physician or health care provider referral is required and that services may vary per location. Hospital programs follow their own intake process, so ask the program directly. Launch Rehab books you directly.
When should I see a doctor before booking physiotherapy?
See your doctor first if you have symptoms that need medical assessment: numbness or weakness that is getting worse, fever with back pain, new bladder or bowel changes, or chest pain with shortness of breath. Severe symptoms call for 911. A physiotherapist screens for these at the first visit and refers you back to your doctor when the findings point there.
What should I bring to my first physiotherapy appointment?
Arrive 10 minutes early to register and bring your Personal Health Care Card number, a list of current medications and your doctor's referral if you have one. For an ICBC claim, bring the claim number and your adjuster's name. For WorkSafeBC, bring your claim number. Wear comfortable clothing, and shorts if the injury is in your lower body.
